Our Discharge Planners can help ease your transition from hospital to home or to another care facility. If you feel you’ll need special assistance once you leave our hospital, please let us know early in your stay so we can put you in touch with one of our Discharge Planners who can provide you with helpful resources. Discharge Planners have information on all types of retirement/assisted living and convalescent hospitals. They can assist in locating help at home through home health agencies, such as DRMC Home Health Services and other local programs.
